Actualización en el manejo del sangrado de intestino delgado

Introducción: clásicamente el sangrado digestivo ha sido dividido como alto o bajo según su origen. El alto corresponde al que se produce en esófago, estómago y primeras porciones del duodeno. El bajo es aquel que se origina en el colon y recto. Quedando el intestino delgado fuera de esta clasificación y por lo tanto sin ser considerado en el diagnóstico. Por esta razón desde 2015 se añade una nueva categoría a las dos anteriores: El sangrado de intestino delgado (SID) o medio. Objetivo: Evaluar las formas de presentación y clasificación del SID. Además de los métodos diagnósticos y el algoritmo de manejo actual. Método: Se realiza una búsqueda de la literatura en la base de datos Medline a través de PubMed, se definen los términos de búsqueda así como de las palabras clave. Resultado: El SID constituye entre el 5 al 10% de las causas de sangrado digestivo. Debe ser considerado en pacientes con hemorragia digestiva o anemia ferropénica cuando los estudios endoscópicos convencionales son negativos. Conclusión: Actualmente con el avance en las técnicas de imagen radiológica, el uso de la video cápsula endoscópica y la enteroscopia profunda se pueden detectar la mayoría de sangrados de intestino medio.
